This website (patriotshare.org) is operated by HIGHER SOURCE LLC, a Florida limited liability company and independent OneShare Health affiliate (“we,” “us”). This policy explains what information we collect on this website and how we use it.
Please don’t send us medical information
We never ask for medical or health information on this website. Please don’t include medical details in our forms, booking page or emails. We’ll go over what matters on a call.
What we collect
- Contact and call-request forms. When you send a form, we receive your name, email address, phone number and message. The forms are processed by Web3Forms, which emails your submission to us at [email protected].
- Booking a call. Our “Book a call” buttons open a Google Calendar appointment page. Google collects the details you enter there (such as your name and email) and sends us the booking. Google’s privacy policy applies to that page.
- Emails you send us. We keep the emails you send to [email protected].
- Basic technical data. This website is hosted on Cloudflare Pages. Like most web hosts, Cloudflare may process technical information such as your IP address and browser type to deliver the site and protect it from abuse.
Cookies and analytics
This website doesn’t use analytics, advertising trackers or cookies of its own. Third-party pages we link to, such as Google’s booking page or OneShare Health’s enrollment pages, may use their own cookies under their own policies.
How we use your information
- To answer your questions, schedule and hold calls, and help you with enrollment.
- To keep basic business records.
We use your phone number to reply to your request. We won’t send you marketing texts or calls without your permission.
Who we share it with
We don’t sell or rent your personal information. We share it only with the service providers that run this website and our email (Web3Forms, Google and Cloudflare), when you ask us to help you apply with OneShare Health, or when the law requires it.
Enrolling with OneShare Health
Applications and memberships are handled by OneShare Health on its own website. OneShare Health’s privacy policy applies to any information you give it.
How long we keep it
We keep messages and booking details only as long as we need them to help you and to keep normal business records.
Your choices
You can ask us to see, correct or delete the information you’ve sent us by emailing [email protected].
Children
This website is meant for adults. Please don’t send us information about a child unless you are the child’s parent or legal guardian.
Changes
If we change this policy, we’ll post the new version here and update the date at the top.
